Admission Policy Statement
The purpose of this facility is to provide a home-like setting for the care of
disabled adults. It is the desire of this organization to provide the least
restrictive environment possible, that will maximize the social and
psychological growth of its residents. The goal of this organization is for the
residents to become as self-sufficient as possible and for their needs to be met
in a dignified and humane manner.
Admission Requirements
1. Persons who require continuous nursing care will
not be accepted. This does not apply to residents
who becomes temporarily ill while in the facility.
2. We will not accept a resident for care unless and
until the licensee has completed a written
assessment of the resident and determined that
he/she is suitable pursuant to all of the following:
The amount of personal care, supervision, and protection required by the
resident is available in the home.
The kinds of services, skills and physical accommodations required to meet the
resident's needs are available in the home.
3. We will not accept a person who requires isolation
or restraint.
4. At the time of admission, and annually thereafter,
a written assessment plan shall be completed with
the resident or the resident's designated
representative, the responsible agent if applicable,
and the licensee. A copy will be retained in the
resident's file.
5. If a resident is referred for emergency admission
and the licensee agrees to accept the resident, a
written assessment plan shall be completed within
15 calendar days after the emergency admission.
The written assessment shall be completed in
accordance with the provisions specified in #2 and
#4 of this admission policy.
